چکیده

R ecently, sleep-disordered breathing and nocturnal hypoxia have been recognized to increase the risk of cerebrovascular disease. Kirkham et al reported that nocturnal hypoxemia was a predictor of future cerebrovascular events in sickle-cell disease. However, it remains unclear whether nocturnal hypoxemic episodes directly lead to nocturnal onset stroke, because other predisposing conditions might confound the association between sleep-disordered breathing and the risk of stroke. Congestive heart failure is often accompanied by central sleep–disordered breathing. We report here a case of nocturnal onset ischemic stroke directly provoked by sleep-disordered breathing, newly developed together with congestive heart failure.

Reversal of nocturnal periodic breathing in primary pulmonary hypertension after lung transplantation.

Nocturnal periodic breathing (PB) closely resembling Cheyne-Stokes respiration in congestive heart failure has been reported to occur in end-stage primary pulmonary hypertension (PPH). We herein describe the clinical course of a 56-year-old female patient with PPH and severe hypoxemia, hypocapnia, and right ventricular compromise in whom sleep-disordered breathing (SDB) resolved after successfu...

Treatment of central sleep apnoea in congestive heart failure with nasal ventilation.

Cheyne-Stokes respiration (CSR) is common in patients with congestive heart failure (CHF). This characteristic crescendo-decrescendo pattern of breathing is often seen during sleep in patients with CHF and is a form of central sleep apnoea. Disordered nocturnal breathing leads to oxygen desaturation, poor sleep quality, and altered sleep architecture. These features may lead to complaints of da...

Continuous positive airway pressure in patients with congestive heart failure and Cheyne-Stokes respiration with central sleep apnea.

With Congestive Heart Failure and Cheyne-Stokes Respiration With Central Sleep Apnea To the Editor: The article by Sin et al1 on the effect and outcome of continuous positive airway pressure (CPAP) in patients with congestive heart failure (CHF) who have Cheyne-Stokes respiration with central sleep apnea (CSR-CSA) is one more step toward further defining the role of treatment for sleep-disorder...

Abnormal breathing during sleep and increased central chemoreflex sensitivity in congestive heart failure.

Central Chemoreflex Sensitivity in Congestive Heart Failure To the Editor: The data of Narkiewicz et al1 expand our understanding of the importance of the imbalance between enhanced central versus peripheral chemoreflex sensitivity in the pathophysiology of congestive heart failure. The authors comment on the links between central cardiovascular and respiratory control, but despite this, patien...

Sleep disordered breathing and its treatment in congestive heart failure.

Sleep disordered breathing (SDB) is a common problem with adverse cardiorespiratory, endocrinological, and endothelial effects. Recent studies demonstrate an even higher prevalence of SDB in congestive heart failure (CHF) than in a randomly selected population, with up to 40% and 11% having Cheyne Stokes respiration-central sleep apnoea and obstructive sleep apnoea-hypopnoea syndromes, respecti...
